6th Edition Task List Self Assessment Form

This Google form can be used once or multiple times to help track progress or identify areas of need for the supervisee.  The link will prompt you to make a copy and requires a google drive account for access.  Once a copy is made you can customize the form to add additional questions to help gather information specific to the individuals you supervise.  To send out the form for completion simply select the send button in the right top corner and enter in the email address for each individual.  Supervisees only need internet access in order to complete the form.  To access their responses simply select the response tab at the top of the screen.  Enjoy!
